Why Patients Consider Dissolving Filler
The decision to dissolve filler is rarely a single moment. It usually develops over time, as patients reassess how their face looks, feels, or photographs. Some have had filler placed years ago and notice it has shifted, settled, or no longer blends naturally with their features. Others are responding to subtle changes such as puffiness under the eyes, fullness in the cheeks that reads differently than it once did, or a contour that has lost its original definition.
A growing number of patients are also choosing to dissolve as part of a deliberate strategy, clearing older product so a new treatment plan can be built on a clean foundation. According to ASPS, the number of minimally invasive cosmetic procedures has grown nearly 200% since 2000, meaning many patients today have a longer cumulative history with filler than ever before, and dissolving has become a natural part of how that history is managed over time.
What Does It Mean to ‘Dissolve’ Filler?
Dissolving filler involves an enzyme called hyaluronidase that breaks the bonds in hyaluronic acid, allowing the filler to be metabolized and cleared by the body. The process is faster than most patients realize. Most hyaluronidase activity occurs within the first 24 to 48 hours, after which the enzyme is rapidly metabolized and cleared from the tissue.

Myth #1: Dissolving Filler Ages You
This is one of the most common fears patients bring up in consultation, and it’s worth addressing head-on. Dissolving filler does not age the face. What it does is return the face to its baseline. The misconception comes from two places.
First, patients have grown accustomed to the volumized version of themselves, so the baseline can feel like a loss. This is a phenomenon described as perception drift.
Second, in patients who have had filler for many years and in larger volumes, the underlying tissue has been stretched, and that’s a separate issue from the act of dissolving itself. In one retrospective review of 157 tear-throughs in 90 patients treated with hyaluronidase, the risk of post-hyaluronidase hollowing was related to the volume and duration of filler use, not the dose or concentration of hyaluronidase. In other words, what patients perceive as “aging” after dissolving is usually a reflection of long-term filler history, not the dissolving process. Patients with smaller volumes and shorter treatment histories generally tolerate dissolving very well.
Myth #2: Dissolving Removes Your Natural Hyaluronic Acid
It’s true that hyaluronidase is not selective. It will break down the HA filler that was placed and also some of the native (natural) HA present in the tissue. But this is far less dramatic than it sounds. The human body contains approximately 15 grams of hyaluronic acid in total, and roughly 30% of facial HA is naturally synthesized and degraded every day through normal metabolic processes. The body is constantly making new HA and there is fast turnover! The temporary, local depletion caused by hyaluronidase is exactly that: temporary and local. Within about 2 to 4 weeks, the body actively regenerates natural HA in the treated area. So while there can be a transient period of looking a bit “deflated” right after dissolving, this is not a permanent state, and it does not mean the skin has been stripped of its natural building blocks.
Why This Topic Causes So Much Anxiety
Filler is personal. Patients see their face every day, and the prospect of any change can feel high-stakes, particularly when stories about poor outcomes circulate widely on social media. Many patients have also invested time and money into their original treatment, which can make the decision to dissolve feel weighty.
In an FDA database review of 10,577 reports covering 29,471 adverse events from injectable fillers dissolving, the most commonly reported events were pain and swelling, which are typically temporary and manageable. Most of the anxiety around dissolving comes from uncertainty about the process itself, which is why a thorough consultation matters.

Both placing and dissolving filler are technique-dependent, anatomy-dependent procedures, and the qualifications of an injector matter enormously.

Dissolving filler does not automatically make the face look older, though it may change how patients perceive their appearance after becoming accustomed to added volume.
