Studies comparing these approaches found that ultrasound-guided treatment used significantly less hyaluronidase, an average of 122.5 units compared to 1,519 units with the traditional “flooding” technique of flooding the entire area with hyaluronidase. Once the filler is mapped out, the skin is cleansed and numbing cream may be applied. Using ultrasound guidance, hyaluronidase is injected directly into the filler deposits while your dermatologist watches placement in real time.

You may feel mild pressure or brief stinging, but most patients tolerate the procedure very well. Dissolving begins immediately and over 24-48 hours, though temporary swelling or redness can occur. Some patients may need multiple sessions, staged a few weeks apart.

Limitations and Considerations

While ultrasound-guided filler dissolving is a powerful tool, it’s important for patients to know its limitations:

  • This applies only to hyaluronic acid fillers: Hyaluronidase works exclusively on hyaluronic acid fillers. Other injectable products, such as biostimulators, cannot be dissolved with this enzyme.
  • Ultrasounds require specialized training. Seeing filler on an ultrasound and correctly interpreting what you’re seeing requires training and experience. Not all providers are trained to identify filler accurately using ultrasound, which is why expertise matters just as much as technology.
  • Not every patient needs an ultrasound: Many filler adjustments can be safely and effectively performed without imaging. An ultrasound is always helpful but not as a requirement for everyone.
