What is “Ozempic Face”?

“Ozempic Face” is a colloquial term used to describe changes in facial appearance after undergoing weight loss using Ozempic (semaglutide), other glucagon-like peptide-1 (GLP-1) receptor agonists, or glucose-dependent insulinotropic polypeptide (GIP) receptor agonists. Due to the often rapid and significant weight loss experienced by patients on these medications, the face may begin to look gaunt or hollow due to loss of fat in the face. In older individuals, this can be accompanied by increased skin laxity and sagging, further contributing to an aged or tired look. Dr. Lisa Ishii specializes in facial changes and support through GLP-1s, and recently answered some of our most common patient questions.

Can Rapid Weight Loss Cause Long-Term Facial Aging Problems?

It is important to note that “Ozempic Face” is not dangerous, but rather a cometic concern. As weight loss continues or is maintained, facial volume loss and skin laxity may progress. Many patients find that although they feel healthier overall, their facial appearance may look older, more tired, or drawn.

Without intervention, these changes can become long-lasting, particularly in older individuals whose skin is less likely to rebound on its own. In more advanced cases, where skin laxity is significant, surgical procedures performed by a board-certified plastic surgeon may be the only option to fully correct the issue.

Are There Preventative Measures for Facial Volume Loss on GLP-1s?

Yes. Prevention and early intervention are key. Before pursuing aesthetic treatments, nutritional optimization should be prioritized. Your prescribing physician or a registered dietitian can ensure you’re maintaining a nutrient-rich diet to support skin and tissue health during weight loss.

Board certified dermatologist Dr. Lisa Ishii recommends that aesthetic treatments be introduced during the weight loss journey, rather than waiting until after weight stabilization. This proactive approach allows us to preserve facial harmony and prevent significant volume depletion that would be harder to reverse later.

For patients who may ultimately require surgical intervention, it’s recommend waiting at least 4–6 months after weight stabilization before proceeding with a surgical referral.

What Non-Surgical Options Can Help Reduce “Ozempic Face”?

An effective rejuvenation plan involves a multimodal approach targeting both the skin and underlying fat compartments. As with weight loss, this process is a journey—results are best achieved with consistency over time.

A combination of hyaluronic gel-based fillers and biostimulators (Sculptra) can be used to restore natural volume to the face for a more youthful contour. For skin laxity, a combination of energy-based devices (ex. Sofwave Ultrasoundradiofrequency microneedling, and laser treatments) and biostimulators (Sculptra, Radiesse) help to stimulate collagen and combat loose skin.
