The neck is often one of the first areas where patients notice visible signs of aging. Changes in this area can feel frustrating, especially when they seem to appear more quickly than expected.

In this blog, Aesthetic Solutions Dermatologist Dr. Payvand Kamrani explains what patients commonly refer to as “turkey neck” and what the non-surgical options are.

What Is a “Turkey Neck”?

“Turkey neck” is a common term people use to describe loose, sagging skin and visible vertical bands in the neck area. These changes can make the neck look less smooth and the jawline less defined. The term comes from the resemblance to the loose skin that hangs from a turkey’s neck.

In reality, what patients call turkey neck is usually caused by several changes occurring at once:

  • Skin laxity due to loss of collagen and elastin
  • Platysmal bands, or visible vertical muscle bands in the neck
  • Loss of jawline definition and a less defined chin-neck angle
  • Submental fullness, or excess fat under the chin

Because multiple structures contribute to these changes, effective treatment often requires addressing more than one layer of the neck.

What Causes a Turkey Neck?

Turkey neck develops from a combination of natural aging, genetics, and environmental factors.

  1. Skin Changes: The skin of the neck is particularly susceptible to aging. Compared with the face, neck skin is thinner and contains fewer oil glands. Research has shown that skin elasticity in the neck can decline significantly with age, dropping from approximately 75% elasticity in younger adults to around 56% in older individuals. Collagen production also declines with age. By the early 40s, collagen production decreases by roughly 1% per year, and these changes are often more visible in delicate areas like the neck.
  2. Muscle Activity and Aging: One of the main contributors is the platysma muscle, a thin sheet-like muscle that runs from the chest up to the jawline. As we age, this muscle becomes more prominent and can separate in the midline, creating the vertical bands often seen when tightening the neck. Studies have shown that platysmal banding is primarily due to muscle activity, not just skin laxity.
  3. Changes in Support Structures: Aging also affects deeper tissues. Fat beneath the chin may accumulate or shift downward, creating fullness and softening the jawline. At the same time, deeper structural support decreases, which can make the angle between the chin and neck less defined.
  4. External Factors: Environmental factors can accelerate these changes. The neck is frequently exposed to sunlight but often forgotten during sunscreen application. Chronic UV exposure breaks down collagen and elastin, contributing to premature skin laxity and wrinkling. Chronic downward head posture (“tech neck”) can also contribute to visible aging in the neck.

Prevention: Can You Slow It Down?

While you can’t stop aging entirely, you can take steps to slow down the visible changes in your neck.

  • Sun Protection: Daily sunscreen is one of the most effective ways to protect the neck. A broad-spectrum SPF 30 or higher should be applied to the neck every morning, just as it is on the face.
  • Medical-grade Skincare: Extending your facial skincare routine to the neck can help maintain skin quality. Products containing retinoids, antioxidants such as vitamin C, peptides and growth factors can help support collagen production and improve skin texture over time.   • Posture Awareness: Constantly looking down at phones and computers places repetitive strain on the neck. While research is still evolving, improving posture and limiting prolonged downward neck flexion may help reduce the formation of neck lines.
  • Early Treatment: Early intervention can sometimes slow visible changes. For example, small doses of botulinum toxin can relax early platysma bands before they become more prominent. An annual laser targeting collagen production can help minimize skin creepiness.

Non-Surgical Treatment Options for Turkey Neck

Many non-surgical treatments allow dermatologists to target different layers of the neck. The best treatment depends on whether the primary concern is muscle bands, skin laxity, excess fat, or a combination of these factors.

Neuromodulator Injections:

Botulinum toxin relaxes the overactive platysma muscle, softening the appearance of vertical bands and improving jawline definition. Large clinical trials involving over 1,000 patients have demonstrated that approximately 77% of patients experienced at least a one-grade improvement in neck bands within two weeks, with results typically lasting three to four months. Treatment involves a series of small injections along the platysma bands and typically requires no downtime. Treatment will also help reduce jowling and improve the jawline structure. In addition, it provides a lift as the muscle being targeted is a depressor, meaning as we relax it with neuromodulators, it cannot pull down on the face.

Injectable Treatments for Skin Quality:

Certain injectable treatments can help improve the overall appearance and quality of the neck by stimulating collagen production and enhancing skin hydration. Some injectables act as biostimulators (Radiesse or Sculptra), encouraging the body to gradually produce new collagen over time. Other injectable treatments (hyaluronic acid gel fillers) are placed more superficially within the skin and focus primarily on improving hydration and skin quality.

Ultrasound (Sofwave):

Ultrasound devices such as Sofwave work wonderfully on the neck. Sofwave is a non-invasive treatment designed to stimulate collagen production in the mid-dermis. Unlike earlier ultrasound technologies that target deeper tissues, Sofwave delivers energy at a precise depth of about 1.5 mm, which is ideal for treating skin laxity and fine lines in the neck. Clinical studies have shown that over 70% of patients experienced visible improvement in skin laxity after treatment, with continued collagen remodeling occurring for up to 12 weeks following the procedure. Treatment is typically performed in a 1-2 sessions with no downtime and it is safe in all skin types!

Radiofrequency Microneedling:

Radiofrequency microneedling combines tiny needles with radiofrequency energy to heat deeper layers of the skin. This stimulates collagen production and gradual tightening. Studies evaluating radiofrequency microneedling for neck laxity have demonstrated significant improvement in skin tightening and contour with high patient satisfaction. Most patients require 3-5 treatments, spaced several weeks apart.

Laser Treatments:

Fractional lasers can improve skin texture, wrinkles, and mild skin laxity in the neck by stimulating collagen remodeling within the skin. Fractional non-ablative lasers create microscopic zones of heat within the dermis while leaving the surface of the skin intact, which triggers new collagen production with minimal downtime. Fractional ablative lasers go a step further by creating tiny controlled columns that remove portions of the skin’s surface while also heating deeper layers, leading to more significant collagen stimulation and tightening. These treatments work by stimulating the body’s natural wound-healing response, gradually improving skin firmness, texture, and fine lines over the following months.

For patients who have both fat under the chin and mild skin laxity, a minimally invasive option is submental liposuction combined with radiofrequency tightening, which we perform at Aesthetic Solutions as Sculpt & Lift. During this procedure, a small amount of fat beneath the chin and along the jawline is removed through tiny entry points using liposuction. After the fat is reduced, radiofrequency energy is delivered beneath the skin to gently heat the tissue. This helps the skin tighten as it heals. The neck rarely ages in just one way. Most patients experience a combination of muscle banding, skin laxity, texture changes, fat accumulation. Because of this, combination treatments often produce the most natural and noticeable results. For example, a patient may benefit from botulinum toxin for platysma bands combined with Sofwave or radiofrequency microneedling for skin tightening, or Sculpt & Lift to remove fat while tightening the skin. When Surgery is the Most Appropriate Path

While non-surgical treatments have advanced significantly, they do have limitations. Patients with significant excess skin, severe muscle separation, or large amounts of deep fat may ultimately benefit more from a surgical neck lift. Surgery can remove excess skin and reposition deeper structures, producing more dramatic and long-lasting results.
