Conversations around fillers have shifted in recent years, with social media often highlighting concerns about overuse. As a result, many people are curious about what options exist beyond traditional fillers.

While dermal fillers remain an important and effective tool, they are one tool within a much broader toolbox of treatments that can support facial aging. In this blog board certified dermatologist Dr. Lisa Ishii reviews alternatives to filler and offers guidance on how to understand which options may be appropriate, along with when filler continues to play a valuable role.

What are Fillers?

“Filler” is an umbrella term that describes a wide variety of injectable agents that are used to rejuvenate the face. Historically, it has included injectable devices such as collagen injections, hyaluronic acid injections, biostimulators, and autologous fat. More recently, “filler” has been used colloquially to refer to hyaluronic acid gel-based filler (HA filler) used to enhance volume of the face.

Why are People Seeking Alternatives to Filler?

HA filler in particular has unfortunately gained a negative reputation due to the misuse and overuse, leading to unnatural results or “filler face.” This has led patients to seek other options for rejuvenation, including regenerative techniques in hopes of longer term results. Additionally, more people are understanding the importance of holistic aesthetic care and how skin quality is just as vital as facial shape – there’s much more to rejuvenation than simply revolumizing and filling lines!

A Review of Non-Filler Options:

Biostimulators (Sculptra, Radiesse):

Versatile options that can help replenish volume and/or enhance collagen production. I love this for patients who have noticed global thinning or “gauntness” of the face that also desire more supple skin quality. This is oftentimes combined with HA filler, if one needs more structure.

Neuromodulators:

Helps with wrinkle reduction, especially the wrinkles that are formed with repeated muscle contraction and movement. This is an injectable medicine that works at the level of the muscle without “filling” the space. In addition to wrinkle relaxing, this category of injectable can enhance brow shape, improve skin texture, sharpen the jawline, and even reshape a square lower face. With consistent treatment, it can help relax even stubborn etched lines.

Lasers and Energy-Based Devices:

Lasers and energy-based devices are extremely wide ranging and can be used to brighten skin tone, smoothen skin texture, tighten crepey skin, or even decrease your future risk of certain types of pre-skin cancers and skin cancers. These are tools I recommend to all of my patients, as smooth and bright skin will help one look more youthful and healthy, even without changing anything else. When is Filler Still the Right Choice?

When used correctly, HA filler and other fillers should be undetectable. It should move with your facial movements. I rarely use this as a solo treatment. HA filler is a powerful tool when mimicking bone or deep fat pockets that we may have lost with time or may have congenitally lacked. A good example of where HA filler shines is along the cheek bone (zygoma) or along the jawline and chin (mandible). Certain anatomic areas respond best to carefully and conservatively placed HA filler, opposed to other treatment options.

How to Determine the Best Approach

A skilled aesthetic provider will always assess your concerns holistically, not simply focus on the singular line or wrinkle that bothers you. Allow your provider to offer a 360-degree assessment to evaluate bone structure, muscle, fat structure, and skin quality. Keep an open mind as you hear about all the treatment options or combination of treatment options to best address your concerns. That being said, at the end of the day, your aesthetic provider should never pressure you into doing treatments that you are not comfortable with. An honest aesthetic provider will candid about expectations or even decline treatments if they are not a good option. Our job is to recommend the best treatment options to meet your goals while educating you on why we are choosing these particular treatments, but being able to pivot and brainstorm alternatives to best fit your values.

There are many effective options to support facial aging beyond traditional fillers, each offering a different way to address changes in structure or skin quality. At the same time, filler continues to be the right choice for certain concerns. Understanding the strengths of each approach can help you make informed decisions and create a plan that feels natural, balanced, and tailored to your needs.
