Laser Treatments for Melanin-Rich Skin: Why PicoSure Pro Stands Out

Laser Treatments for Melanin-Rich Skin
By: Payvand Kamrani, DO | Cosmetic Dermatologist

Laser treatments have come a long way, but for those with melanin-rich skin, concerns about hyperpigmentation, scarring, and burns are still very real. Traditional lasers often carry risks for darker skin tones, but the PicoSure Pro laser is changing that narrative. Designed with all skin types in mind, board certified dermatologist Dr. Payvand Kamrani covers all you need to know about the PicoSure Pro.

Understanding the Challenge: Laser Treatments and Melanin-Rich Skin

Laser treatments can be riskier for individuals with higher levels of melanin. This is because melanin more readily absorbs laser energy, which can increase the likelihood of side effects such as:

  • Post-inflammatory hyperpigmentation (PIH)
  • Hypopigmentation (light spots)
  • Scarring
  • Burns

These complications are more common in darker skin types. For example, traditional Q-switched lasers, which were used before the new PicoSure Pro laser, can cause post-inflammatory hyperpigmentation (PIH) in up to 25% of patients with skin of color. Unfortunately, many people have real experiences of laser burns and complications when treated with the wrong devices, see an example here.

How The PicoSure Pro Differs

PicoSure Pro uses advanced picosecond technology, delivering energy in extremely short bursts (one trillionth of a second!!). Instead of relying on heat, this laser uses a photomechanical effect to break up pigment. By minimizing heat, it reduces the risk of harming surrounding skin, which makes it safer for melanin-rich skin compared to older lasers.

While other devices, such as the Clear + Brilliant laser, can also be used safely on darker skin tones at lighter settings, PicoSure Pro offers a more powerful option for stubborn pigmentation with an excellent safety profile. It combines effectiveness with precision, giving patients with darker skin tones access to results that were once harder to achieve safely. – Dr. K

Conditions Treated with the PicoSure Pro in Melanin-Rich Skin

PicoSure Pro has shown effectiveness in treating several pigmentation-related conditions common in melanin-rich skin, including:

  • Melasma
  • Post-inflammatory hyperpigmentation
  • Freckles and sun spots
  • Café-au-lait macules
  • Certain types of birthmarks (e.g., Nevus of Ota)

Beyond pigmentation, PicoSure Pro can be used with a special fractionated handpiece to rejuvenate the skin by improving texture, reducing fine lines, and softening the appearance of acne scars.

PicoSure Pro is considered a gold standard for tattoo removal, since it works by breaking down unwanted ink in much the same way it targets pigment in the skin. This combination of safety and effectiveness makes it one of the most reliable options available for patients with darker skin tones.

What Patients Can Expect

Before Treatment:
Avoid sun exposure or tanning for at least 2–4 weeks before your session. Some topical products may also need to be paused before treatment, based on your provider’s guidance.

During Treatment:
Most patients describe the sensation as a brief snapping or tingling. A topical numbing cream is typically applied about an hour beforehand, and cool air may be used to reduce discomfort. Sessions usually take between 15 and 30 minutes.

After Treatment:
Mild redness and swelling are common within the first 24–48 hours. Patients can usually resume normal activities the same day, though strict sun protection is strongly advised. Pigmented areas may darken temporarily before fading as part of the natural healing process.

Results:
Visible improvement tends to appear gradually over several weeks, with optimal results often achieved after multiple sessions, depending on the condition being treated.

Key Questions to Ask Before Any Laser Treatment on Melanin-Rich Skin

Regardless of the device being used, it’s important for patients with melanin-rich skin to be well-informed before undergoing laser procedures. Consider asking the following:

  • Does my provider have experience treating patients with my skin tone?
  • What laser will be used, and is it appropriate for my skin type?
  • How many sessions are expected for the desired outcome?
  • What are the specific risks and side effects for my skin?
  • What should I avoid before and after treatment?
  • How important is sun protection during the healing process?

Asking these questions can help ensure that the treatment plan is safe, evidence-based, and tailored to your needs.
