What are the most common undereye concerns?

That worn-out look often comes from more than one reason, which is part of what makes the undereye area so tricky to treat. The usual culprits are dark circles (a darkening or discoloration of the skin beneath the eye), hollows or tear troughs (a sunken groove where the lower lid meets the cheek), puffiness or bags (fullness caused by fat that has drifted forward), fine lines and crow’s feet, and changes in skin quality such as crepiness and looseness.
Dark circles are especially common. In a global survey of nearly 48,000 adults, roughly 15 percent reported dark circles under the eyes – one of the most common concerns anywhere in the world. There is a lot of understand about the undereye: these concerns can look nearly identical on the surface but stem from very different causes underneath. That’s exactly why they don’t all respond to the same treatment.
How does the natural aging process contribute to these?
Aging affects the undereye area on every level: bone, fat, muscle, and skin. Over time, the bone around the eye socket slowly recedes and the socket widens, taking away some of the support your lower lid depends on. As that support gives way, the small fat pads that cushion the eye can slip forward, creating the look of bags or puffiness.
At the same time, your skin is losing collagen and elastin — partly from natural aging, partly from years of sun exposure. As it grows thinner and looser, the blood vessels and pigment underneath become more visible (a common source of dark circles), and fine lines and crepey texture start to appear. Meanwhile, lost volume in the midface deepens the tear trough, adding to that hollow, shadowed look.
And these changes aren’t reserved for later in life. They can begin as early as your 30s, which is why so many patients are surprised to see undereye changes sooner than they expected.
