Patients exploring injectable treatments are often surprised to learn that not all products work the same way. While many products are grouped together under the umbrella of “injectables,” different categories are designed to address different goals!

What Are Dermal Fillers?

Dermal fillers are injectable, gel-like substances, most often made from hyaluronic acid (a sugar molecule your skin already produces naturally). They work by physically restoring volume where it’s been lost, whether that’s a deeper fold along the cheek, thinning lips, or a hollowed-out tear trough.

Because hyaluronic acid is something your body already makes on its own, the risk of an allergic reaction is very low. Results show up quickly and can appear same day, but typically take 2-4 weeks in order to allow the product to settle. The technology also has a strong track record: hyaluronic acid fillers have been FDA-approved in the United States since 2003, and today there are more than a dozen HA products on the market, each formulated for specific areas like the lips, cheeks, tear troughs, or jawline. Brands you’ve probably heard of include Juvederm, Restylane, and RHA.

What Are Biostimulators?

Biostimulators work in a completely different way. Rather than ‘filling a space’, they kickstart your own collagen production over time, a process called neocollagenesis. The two most well-known biostimulators are poly-L-lactic acid (PLLA), better known as Sculptra, and calcium hydroxylapatite (CaHA), or Radiesse.

Once injected, they act like a scaffold that wakes up the fibroblasts in your skin and tells them to start making fresh collagen and elastin. The trade-off is patience. You won’t see a dramatic change overnight the way you might with a filler. Results build gradually over weeks and months, but they also last longer, which is part of why patients tend to be so happy with the outcome. A recent meta-analysis pulled data from multiple studies and found a 91% satisfaction rate with biostimulator treatments.

What Types of Concerns Do Biostimulators vs. HA Fillers Treat?

Dermal fillers are the go-to for targeted, precise corrections: softening a nasolabial fold, plumping the lips, filling a tear trough, lifting the cheeks, or sharpening the jawline. They give you immediate, focused volume in a defined spot, and the results stick around. Studies show HA fillers can maintain 50% to 86% of their original volume at 24 months, depending on the product and area treated.

Biostimulators are better suited for broader, more diffuse concerns: overall volume loss across the face, skin laxity, crepey texture on the body, that general thinning we all start to notice with age. Instead of filling a line or fold, they improve the quality of the skin itself, the firmness, the elasticity, the way it bounces back when you smile.

Can Fillers and Biostimulators Be Combined?

Yes, and combining them often delivers the best results. The two work through different biological mechanisms, which means they complement each other rather than competing. A typical example: a patient might come in for Sculptra to rebuild foundational collagen and improve overall skin quality, then add a hyaluronic acid filler later to precisely sculpt the lips or cheeks. The end result is a natural-looking canvas with targeted definition.

What Factors Guide Product Selection?

It depends on the patient! Every face is different, every set of goals is different, and the right plan really comes down to the consultation. Some patients come in wanting to address one very specific concern, like fuller lips or a softer nasolabial fold, and a hyaluronic acid filler is exactly the right tool for that. Others are looking at the bigger picture: overall thinning of the skin, a flatter midface, that general “tired” look that’s hard to pinpoint, and that’s where a biostimulator like Sculptra really shines.

Here’s what I tell most of my patients: aging doesn’t happen in just one way. We lose volume, yes, but we also lose collagen, our skin gets thinner, and the underlying structure of the face shifts over time. Because of that, the most natural-looking results usually come from using both categories together, with each one addressing a different facet of what’s changing.

A biostimulator can rebuild the foundational collagen and improve skin quality over the long haul (Sculptra, for example, has been shown to maintain its effects for up to 25 months), while a filler adds precise volume and definition exactly where you want it. It is typically a combination of the two that we use in the office. Location of treatment, your timeline, your personal preferences, all of it factors in and the best plan is always the one tailored specifically to you.

In the case of biostimulators vs. HA fillers – they aren’t always competing treatments! In many cases, they work best when used together as part of a thoughtful, individualized plan. When combined strategically, these treatments can work synergistically to support balanced, natural looking results.
